Coincidentally I posted on this topic on my blog just before going on

In simple terms, I would have thought that the MoQ would agree with the
Szasz-type rejection of conventional understandings of mental illness.
Specifically, the division between levels 2 and 3 in the MoQ would suggest
that the methods and analyses suitable for treating low quality at level 2
*cannot* be directly transferred to deal with low quality at levels 3 or 4.
Which provokes interesting questions about what can - but that leads onto
religious questions....
